What Couple’s Fertility Investigations Almost Always Miss

In my 12 years of clinical fertility practice, the patterns that appear in couples presenting for a joint review are remarkably consistent. And the gaps are almost always the same.

Sperm DNA fragmentation — the most consistently missed investigation

Standard semen analysis checks count, motility, and morphology. When these return within normal ranges, male factor fertility is usually considered ruled out.
But normal semen analysis does not mean healthy sperm DNA. Sperm DNA fragmentation — the integrity of the genetic material inside each sperm — is directly linked to failed fertilisation, poor embryo quality, recurrent miscarriage, and repeated IVF failure. It is rarely tested. Even after multiple failed cycles.
In my clinical experience, high sperm DNA fragmentation with completely normal semen analysis is one of the most common findings in couples who have been told there is no male factor issue.

Systemic health factors for the male partner — almost never assessed

Beyond sperm DNA, the factors that affect sperm health systemically are almost never investigated in conventional fertility care:

Chronic sleep disruption elevates cortisol and reduces testosterone — directly affecting sperm production and DNA integrity. Excessive high-intensity exercise or heavy protein powder use can suppress testosterone and increase oxidative stress on sperm cells. Gut health and nutrient absorption affect the micronutrients sperm development depends on — zinc, selenium, CoQ10, folate. Environmental toxins and endocrine disruptors accumulate in male reproductive tissue. These are findable. They are addressable. And they are almost never part of the conversation.

The Truth About Male Fertility That Most Clinics Don’t Say Clearly Enough

Male factor fertility contributes to approximately 40-50% of fertility challenges. This is widely acknowledged in the clinical literature.

And yet the male partner is almost always the secondary focus of fertility investigation. A semen analysis — often a single one — and if the numbers are broadly acceptable, male factor is considered addressed.

It isn’t.

Sperm DNA fragmentation can be significantly elevated with completely normal semen analysis numbers. High-intensity exercise protocols — particularly excessive cardio or heavy weightlifting — can suppress testosterone and increase oxidative damage to sperm. Protein powders and some sports supplements contain compounds that affect testosterone and reproductive hormone balance. Poor sleep reduces testosterone production and increases the cortisol that competes with it. Gut health affects the absorption of every micronutrient sperm development depends on.

None of these are assessed in standard fertility care. All of them are addressable.

Do both partners need to attend?

Yes — a Couple’s Fertility Review is specifically designed for both partners to attend together. Individual assessments are available for each partner within the session, as well as a joint review. Both partners attending together is what makes this review fundamentally different to individual consultations.

My partner is resistant to the idea. Is that common?

Very common — and understandable. Men are often less comfortable in clinical fertility settings, particularly when they feel their contribution has been minimised or overlooked. In my experience, once both partners are in the consultation together, the male partner’s engagement almost always increases — because for the first time, someone is taking their health as seriously as their partner’s. My partner's semen analysis was normal. Does he still need to attend?

Yes — normal semen analysis does not mean optimal sperm health. Sperm DNA fragmentation, systemic health factors, lifestyle contributors, and Ayurvedic constitutional assessment are all independent of semen analysis results. In my clinical experience, the most significant male factor findings often come in cases where semen analysis was completely normal.

We've already had individual consultations. Is a joint review still useful?

Yes — because a joint review looks at both partners’ pictures together in a way that individual consultations cannot. The interaction between both constitutional types, the coordination of both protocols, and the shared lifestyle and environmental factors are only visible when both partners are assessed simultaneously.

Is this available online?
