Why ‘Unexplained’ Is Almost Never Truly Unexplained
In 12 years of clinical fertility practice, I have never seen a case of genuinely unexplained infertility — once I’ve looked properly.
What I see, consistently, are cases where the explanation exists — but wasn’t found, because the investigation didn’t go far enough.
The sperm factor — the most consistently missed piece
Standard semen analysis checks three things: count, motility, and morphology. When these come back within normal ranges, the conversation about male factor fertility is usually closed.
But normal semen analysis does not mean optimal sperm health. And it tells you nothing about sperm DNA fragmentation — the integrity of the genetic material inside each sperm. High sperm DNA fragmentation is directly linked to failed fertilisation, poor embryo quality, early pregnancy loss, and — critically — infertility that appears completely unexplained on a standard panel.
Sperm DNA fragmentation testing is not routinely offered. In many cases, it isn’t offered at all — even in private fertility clinics. And when a couple is told ‘semen analysis is normal, so male factor isn’t the issue’ — that conclusion is based on incomplete information.
The medical response to this gap is often: ‘You can always do IVF or ICSI.’
As if IVF or ICSI is a simple, straightforward decision. As if it costs nothing — financially, physically, emotionally. As if it is the obvious answer to a question that hasn’t actually been properly investigated yet.

My partner's semen analysis was normal. Does sperm health still need to be assessed?
Yes — significantly. Normal semen analysis does not mean normal sperm DNA integrity. High sperm DNA fragmentation with completely normal semen analysis is one of the most consistently missed causes of unexplained infertility.
